What Is Chronic Respiratory Failure?

Chronic respiratory failure occurs when the respiratory system fails to either:

  • Take in enough oxygen (known as hypoxemic respiratory failure, this leads to low blood oxygen levels)
  • Remove enough carbon dioxide from the blood (known as hypercapnic respiratory failure, this leads to high carbon dioxide levels)

Unlike acute respiratory failure—which occurs suddenly and typically requires emergency treatment—chronic respiratory failure develops gradually and worsens over time. Symptoms may include:

  • Difficulty breathing
  • Shortness of breath
  • Rapid or extremely slow breathing
  • Coughing
  • Wheezing
  • Fatigue
  • Confusion
  • Anxiety
  • Persistent headache
  • Irregular heartbeat
  • A blue tint on the skin, lips, or fingernails

What Causes Chronic Respiratory Failure?

Chronic respiratory failure usually occurs when the airways narrow or sustain damage, thereby limiting how much air can move in and out of the body. Some of the conditions that can lead to chronic respiratory failure include:

  • Amyotrophic lateral sclerosis (ALS)
  • Chest injuries
  • Chronic obstructive pulmonary disease (COPD)
  • Cystic fibrosis
  • Muscular dystrophy
  • Pneumonia
  • Scoliosis
  • Spinal cord injuries
  • Strokes

How Is Chronic Respiratory Failure Treated?

Chronic respiratory failure treatment will vary from one patient to another, but may include:

  • Treatment for the underlying condition
  • Medication
  • Oxygen therapy
  • Ventilation
  • Tracheostomy
